Federal Deployment Forms

  • Anyone interested in deploying through FEMA (ie- to the Gulf Coast for Katrina Relief Efforts) should complete/submit the following checklist. Items 1-13 are available online. These must be printed out, signed and mailed to MTMRC.
  • Those interested in federal deployment must complete the Federal Deployment Application Checklist as well as submit all basic member information prior to or concurrently with application for federal deployment
  • Please e-mail mtmrc@vanderbilt.edu with your home address to request a fingerprint card packet including items 14-19.
  • Please print off the Federal Deployment Application Checklist and Instructions and review instructions carefully prior to completing the packet.

  18. Photo (passport-style and/or head/shoulders digital). This can be obtained for a small fee at any local store with photo service: CVS, Walgreens, or Wal-Mart)
